The control of pH in body fluids and subcellular compartments is achieved by a number of biological buffers. The most important buffers in physiological systems are bicarbonate, phosphate, and proteins. Buffers consist of a weak acid and its conjugate base that are in equilibrium and are able to donate and accept protons from the surrounding medium. The relationship between pH, pKa and the concentration of the conjugate acid and base of a buffering system is described by the Henderson-Hasselbach equation. Disturbances in the acid-base balance can lead to acidosis, a plasma pH less than 7.35, or alkalosis, a plasma pH greater than 7.45.
