Annotation: It would be an interesting subject for a medical historian, how an important area of medicine could become so neglected. This is just the case with the spasm of smooth muscles. After the discovery and successful use of papaverine, all the following drugs have the disadvantage of a rich side effect profile despite the fact they were chemically clarified and toxicologically tested. The role of smooth muscle relaxants, however, has not changed for several decades: surgical intervention underwent tremendous development, i.e. catheterisation, laparoscopic surgery, etc. but the treasury of drugs to decrease or relieve the extreme pain of spasm, did not change significantly. Epidemiological data show that the number of patients suffering cholelithiasis, stones of the kidney and the ureter, significantly increased in parallel with the improvement in the reliability of investigational methods and epidemiological surveys in developing countries. The development of gall-bladder and kidney stones is strongly linked to food habits, genetic and environmental factors, etc. Therefore the necessity for the immediate and effective treatment of colic states in undeniable; but we should understand that these types of treatment are not causal but very important regarding the relief of pain and the quality of life of the patient. In this respect drotaverine is one of the most successful drugs, but in spite of its presence on the market for many years its exact therapeutic field has not been clarified even though it was the drug of choice in colic states. The originator of drotaverine is Sanofi-Chinoin Pharmaceutical and Chemical Works Ltd. and they welcome the opportunity to summarise present knowledge on the management of smooth muscle spasm, and to put drotaverine in its well-determined therapeutic lace in the management of this common clinical situation. We hope that this book will guide our colleagues on the diagnostic tools, in differential diagnoses and in the treatment methods of smooth muscle spasm management on the basis of current theoretical and practical research
