Annotation: The aim of this study was to determine the rates of recurrences of stage I endometrial cancer (EC) and features of their localization depending on the clinical and pathological characteristics of the tumor and methods of patients’ treatment. Patients and Methods: The study included 968 patients with stage I endometrioid EC, who underwent surgical treatment in the Department of Oncogynecology of the National Cancer Institute in 2015–2019. Surveillance of patients lasted from January 2015 to December 2020, with a minimum follow-up period of 1 year from the date of surgery. Adjuvant radiation or chemotherapy was performed depending on the clinical and pathological characteristics of the EC case. Results: During the follow-up period, recurrences were observed in 7.0% of cases and were most often found in stage IC of low differentiation grade. It was found that during surgical treatment without adjuvant therapy relapses occurred in 12–36 months after the start of treatment, with adjuvant radiation therapy — in 6–18 months, and with adjuvant chemotherapy — in 32–60 months. Recurrences most often occurred in patients with EC who underwent surgical treatment in combination with chemotherapy (p 0.05). The lowest number of recurrences was recorded among patients who underwent surgery as an only treatment. The best 5-year survival rate was observed in the group of patients with surgical treatment (93%), and the worst — in the patients treated with combination of surgery and chemotherapy (57%). In patients without recurrences, the survival rate after treatment was 97%, while in patients diagnosed with relapses, the survival rate was 65%. Conclusion: Despite the predominantly favorable course of EC stage I, some patients develop relapses. The rate and localization of recurrences depend on the histological structure of the tumor and treatment regimens of the EC patients

Annotation: Background. The problems of pre-operative cancer diagnosis of the fallopian tube are associated with a rare occurrence of this disease. More rarely, there is a combination of the fallopian tube cancer and other gynecological tumors. Clinical observation of the fallopian tube cancer and endometrial cancer in postmenopausal woman is presented, which demonstrates the difficulty of the pre-operative diagnostics of these pathologies combination. Case presentation. We described a case of uterine tube cancer in Ukrainian woman (77 years), which had been combined with adenocarcinoma of an endometrium. Conclusion. Primary fallopian tubes carcinoma is a rare disease with an insufficiently studied etiology. The clinical manifestations are not always present in full. Diagnosis in pre-operative period is very hard to establish. Only the qualitative morphological investigation allows establishing the primary lesion of the fallopian tube. Introducing immunohistochemical investigations in practice serves as additional methods for correct diseases diagnostic, which allow more fully characterizing the prognosis and suggesting appropriate treatment of the patients.

Annotation: Для определения тромбоопасности онкогинекологических больных было осуществлено комплексное динамическое исследование состояния системы гемостаза у 87 гинекологических больных и для сравнения у 30 практически здоровых женщин, которые проходили профилактическое обследование. Все больные были госпитализированы в отделение хирургии Центра реконструктивной и восстановительной медицины УК «ОНМедУ» за период 2016– 2018 гг. для проведения планового хирургического вмешательства. Исследование системы гемостаза имеет первостепенное значение для диагностики различных видов кровоточивости, тромбофилических состояний, в том числе при критических состояниях. Динамический контроль гемостаза необходим и при проведении антитромботической терапии в процессе консервативного и хирургического лечения различной патологии. Для получения комплексной информации о функциональном состоянии компонентов системы гемостаза приходится использовать большое количество разнообразных методик. При этом нет единого мнения о минимуме тестов, позволяющих решить данную проблему. Проведение комплексного исследования системы гемостаза в условиях повседневной клинической практики вызывает серьезные затруднения
Haemostatic potential in cancer patients is characterized by pronounced functional changes in almost all links in the system of hemostasis and fibrinolysis. Violations of the functional state of the hemostasis system that develop in oncopathology, are implemented in thrombohemorrhagic and thromboembolic complications and play a significant role in the course and outcome of the disease. The above changes indicate the development of chronic disymed intravascular coagulation in cancer patients. Objective of the study. The purpose of this study is to evaluate the initial state of the hemostasis system in patients with endometrial cancer, the study of the functional state of vascular-platelet, coagulation units, and fibrinolysis of the hemostasis system and their disorders using low-frequency vibration piezoelectric thromboelastography (NPTEG). Materials and methods. The initial state of the vascular-platelet, coagulation and fibrinolysis of the hemostasis system, the hemostasis system was studied in 87 patients undergoing treatment for endometrial cancer in the department of surgery of the Center for Reconstructive and Rehabilitation of the Medical Center “ONMedU” for the period 2016–2018 for scheduled surgical intervention. Results. The results of the studies represent a group of endometrial cancer patients as a model in which are pronounced disorders of the aggregate state of the blood, where hypercoagulation dominates the anticoagulant potential. Inadequately enhanced activation of one or both coagulation units, even with the normal functioning of the anticoagulant system, can lead to generalized thrombus formation. Therefore, it is necessary to take into account the prevalence of thrombogenesis processes over fibrinolysis processes in this patient population and to consider the activation of platelet aggregation as an additional factor that participates in providing mechanisms for hypercoagulability
