Анотація: Current real-time science and technology techniques based on ENDORET PRGF technologies and the implementation of their results in practical medicine, provide ample opportunities for the use of cell autotransplantation for guided tissue regeneration, including in the treatment of "disuse atrophy" of the human lower jaw bone tissue with early tooth loss. The purpose of this work is to ensure the principles of biological feasibility and physiological capacity, technical rationality. The pathology in the distal segment of a lower limb, which is a vertical atrophy of bone tissue was confirmed by the methods of computed tomography, Vatech PaX-I 3D Green systems of extra-oral radiography with a scan size range of 16×9 cm, a focal spot of 0.5 mm (IEC60336) with a gray scale of 14 bits with a size of 0.2/0.3 voxel. Using ENDORET PRGF technology, according to the approved BTI protocol, autocellular grafts that have provided a positive result that meets the basic principles of the goal in restoring the mechanisms of physiological processes of normal quantitative and qualitative morphology of bone tissue, with its biological characteristics were obtained. The results of the study showed that bone tissue on a scale of shades of gray based on the classification of Hounsfield, is not characterized by a single biotype and in the area of missing 3.6 tooth belongs to the second biotype, and in the area of missing 3.7 tooth – to the first biotype by its density. The indicators of densitometric determination confirmed excessive mineralization of the trabecular layer, ie vertical atrophy of bone tissue, which is in the sagittal section in the projection of the missing 36 teeth – with a maximum number of 881 gray standard units (GSU), M=315 GSU (where, M is the average value of absolute number); sagittal section in the projection of the missing 37 teeth – with a maximum number of 1726 GSU, M=1173 GSU. This clinical experience with the use of autocellular grafts in the treatment of "disuse atrophy" of the bone tissue of the jaws, which is essentially scientifically-research in nature, based on modern, at the same time available technologies of cell engineering and technical progress, provides a predictable result of clinical observation and deserves further research and practical testing

Анотація: Burns, as a stress factor, cause circulatory and microcirculation disorders in the organs. In the scientific literature, the issues of structural reorganization of the vascular bed of the adrenal glands in thermal trauma and under conditions of correction by lyophilized xenoskin are insufficiently covered and therefore require further study. The aim of our study was to study the microscopic changes of the vessels of the adrenal glands in the dynamics after experimental thermal trauma and under conditions of correction. The experiment was performed on adult white male rats. Grade III burn (18-20 % of the skin surface) was applied under thiopental-sodium anesthesia with copper plates heated in water to a temperature of 97-1000C, on the epilated surface of the skin of the back for 10 seconds. One day after the thermal injury, an early necrectomy was performed and the wound was covered with a substrate of lyophilized xenoskin. Animal decapitation and microscopic changes were studied at days 7, 14, and 21 of injury modeling. The manufacture of histopreparations was carried out according to conventional methods. Histological examinations after experimental thermal trauma and correction of xenoskin substrate on the 7th day showed that already in this period of the experiment there were less pronounced destructive changes in the structure of the vessels of the adrenal glands. Some blood vessels were unevenly filled and dilated. The hemocapillaries of the microcirculatory tract are blood-filled, the vessel wall is destructively altered, and perivascular edema is present. On the 14th day of the experiment, moderately blood vessels were observed. The wall of large vessels is slightly swollen, partially thickened, hemocapillaries are moderately blood-filled, with dilated lumens and continuous endothelial lining. Under the conditions of application of the substrate of lyophilized xenoskin on the 21st day of the experiment, the structural organization of the vascular bed of the adrenal glands was significantly better compared to the previous terms of the study. Most vessels had evenly dilated lumens and moderate blood supply, and wall edema was insignificant. The walls of microvessels are preserved, well structured, endotheliocytes without severe damage, without signs of infiltration and edema. Thus, the results of microscopic examination showed that early necrectomy, and subsequent closure of the wound with a substrate of lyophilized xenoskin, creates optimal conditions for regenerative processes in both burns and vascular bed of the adrenal glands. On the 7th day after the burn and under conditions of correction, the study of the vascular bed of the adrenal glands showed a moderate degree of destructive changes in the vascular bed of the adrenal glands and the initial signs of their regeneration. In the late terms (14-21 days) of the experiment the relative normalization of cellular parenchymal components of the organ was established against the background of restoration of the structural organization of the microcirculatory tract

Анотація: Multiple sclerosis is a demyelinating, degenerative disease that is associated with increasing disability. Multiple sclerosis is one of the main reasons for the permanent disability of young people. To improve the quality of life there is a need for comprehensive rehabilitation measures, especially in the initial stages of the disease. In recent years, there are new techniques of non-invasive neuromodulation that can improve the patient’s recovery. Methods that can affect neuroplasticity include transcranial magnetic stimulation. The aim of this study was to assess the safety and potential for adverse effects of transcranial magnetic stimulation during and after a session in patients with multiple sclerosis. The results were obtained by examining 80 patients with multiple sclerosis (diagnosed according to the criteria of McDonald et al., 2017), who received a course of rTMS. All patients received disease-modifying therapy drugs, taking into account the recommendations of the European MS Treatment Guidelines Released. For the purpose of noninvasive neuromodulation in the process of treatment and rehabilitation, rTMS was used. The course of treatment was performed on a MagVenture device, MagPro X100 and an 8-shaped inductor (coil). All patients were tested using a safety questionnaire and selected according to the indications and contraindications of rTMS. Depending on the leading clinical manifestations, different protocols for rTMS were used. Protocols with high-frequency stimulation of DLPFC zones and primary motor cortex, M1 zone were selected. Statistical data processing was performed using the licensed office suite Microsoft Excel 2013. In some patients with multiple sclerosis (35.0 %) during rTMS, during the session or the day after the session, and in some cases later, adverse events were noted. Depending on the stimulation zone, the frequency of their occurrence was different. When using high-frequency rTMS of the primary motor cortex, adverse events were observed in 32.5 % of cases in the form of discomfort in the area of the procedure, headache, changes/decrease in hearing, neck pain, episodic paroxysmal anxiety. When using high-frequency rTMS DLPFK, adverse events were observed in 33.8 % of cases in the form of discomfort in the area of the procedure, headache, hearing loss/transient changes, transient changes in cognitive function, neck pain, high blood pressure, shooting pain in the temporal area on the stimulation side. According to our observations, adverse events were observed not only during rTMS, some of them continued to be observed during the day after the session and more. Thus, TMS occupies an important place in the process of non-drug rehabilitation of patients with multiple sclerosis. The safety of TMS includes careful selection of patients in accordance with the criteria of indications and contraindications of the procedure, the patient’s consent to TMS, the choice of stimulation protocol with personalized selection of parameters, prevention of side effects and monitoring of the patient during TMS

Анотація: The problem of burns is relevant in the world and in Ukraine. Thermal injury is not limited to local tissue changes, but causes significant morphofunctional disorders in the organs and systems of the affected organism, the result of this factor is a violation of blood supply to organs, including the pancreas. The aim of this work was to establish the histological condition of the vessels of the pancreas after experimental thermal injury. The experiments were performed on 40 adult white rats. Burns were applied under thiopental-sodium anesthesia with copper plates at a temperature of 96-97℃ on the epilated surface of the skin of the back of animals for 15 seconds (the lesion area was 18-20 % of the body surface). Decapitation of animals was performed on 1, 7, 14 and 21 days of the experiment. Pieces of the pancreas were taken for histological examination and processed according to generally accepted methods. Histological sections 5-6 μm thick were stained with hematoxylin-eosin, impregnated with silver by the Gordon-Sweets method, as well as staining by the tricolor method of MSB (ORB). Histological specimens were examined and documented using an SEO SCAN light microscope and a Vision CCD Camera. In the early stages of the experiment revealed a violation of the blood supply to the body. The lumens of most arteries were found to be dilated, and the wall thickness increased due to hypertrophy of the smooth muscle cells of the media. Leukocyte infiltration was determined in the perivascular spaces. The lumens of the small and middle veins were dilated and filled with blood, with parietal thrombi in some of them. Capillaries are characterized by a moderate expansion of their lumens, stasis. There was swelling of the paravasal spaces. Significant destructive-degenerative, inflammatory and sclerotic changes were observed in the later stages of the experiment. The walls of the vessels were moderately swollen. Smooth myocytes of the arterial media were hypertrophied, the cytoplasm was vacuolated, and contained pyknotically altered nuclei. Adventitia of vessels, especially veins, was swollen and fibrous. Occasionally there were violations of the wall of the components of the microcirculatory tract, which was accompanied by hemorrhage. There are single thrombosed vessels, spasmed arterioles, hemocapillaries with dilated lumens. The greatest changes, especially on the 21st day, were the adventitial membrane with the growth of collagen and reticular fibers. The blood supply to the vessels was moderate. Vessels of small diameter and hemomicrocirculatory tract were also significantly altered, their wall is deformed, thin, homogeneous. The lumens are dilated, with the formation of blood clots. Thus, in the early stages of the experiment there are adaptive-compensatory processes and initial destructive changes in the walls of the vessels of the organ, manifested by stasis, thrombosis, edema, deformation, infiltration of the wall and perivascular space. In the late period, significant destructive-degenerative changes in the structure of the walls of the vessels of the pancreas, accompanied by hemorrhage, inflammation, sclerosis of the wall and edema of the adventitia. These changes lead to a deterioration of blood supply and, consequently, a decrease in the functional activity of the body

Анотація: The thyroid gland is important for the normal functioning of the body, and is the largest endocrine organ, which among the endocrine glands will be the first in the process of embryogenesis. In recent decades, the prevalence of thyroid pathologies of various origins among the world’s population has reached critical proportions. The use of glucocorticoids during pregnancy remains a debatable issue in obstetrics today, as they can both positively and negatively affect the processes of organ morphogenesis and be the cause of pathological conditions in the postnatal period. It is known that the entry of cortisol from mother to fetus through the placenta is controlled by enzymes produced by the latter. However, synthetic glucocorticoids, such as dexamethasone, can freely cross the blood-placental barrier and cause changes in postnatal immunity and disease in the future. The thyroid gland, having a relatively "simple" basic structure depending on the level of activity and the period of ontogenesis, exhibits various forms of morphological organization. Ideas about the structure and function of the thyroid gland were formed in the course of centuries of its study. Modern experimental and technical capabilities of the study of the body provided information about the structural and functional features of the thyroid gland and the numerical relationships of its structures at different levels of the organization. The aim of the study was to establish the features of the morphogenesis of the rats’ thyroid gland from 60 to 120 days of life after prenatal exposure to dexamethasone. Statistical analysis of the results was performed using a personal computer based on the Windows XP operating system using the statistical package "Statistica for Windows 6.0" (StatSoftInc., Serial number №AXXR712D833214FAN5), Excel (Microsoft Office, USA). Methods of variation statistics were used. All research results were recorded in journals and protocols of primary documentation, as well as with the use of electronic media. Significance of differences between groups was assessed using Student-Fisher t-test for a confidence level of at least 95 % (p 0.05). During the study it was found that in the juvenile period the morphological structure of the rats’ thyroid gland of intact and control groups is finally formed, in the parts of which the peripheral and central parts with rather high sclerosing index (6.20 and 6.46, respectively) can be microscopically distinguished due to prevalence. the percentage of parenchymal component (60.82±1.13 % and 61.44±0.71 %) above the stroma (9.86±1.02 % and 9.53±0.94 %). The study of material obtained from animals prenatally exposed to dexamethasone showed a violation of the morphogenesis of histoarchitectonics of the thyroid gland with the formation of a lobular structure of its particles due to an increase in stromal component (23.63±0.88 %), which was expressed in a decrease in sclerosis index (2.364), but there was a compensatory increase in the percentage of follicular epithelium on the 120th day of life (55.87±0.79 %) compared with the 90th day (49.24±1.25 %), without morphological signs of functional disorders.

Анотація: Thermal injury is a lesion caused by thermal agents. With deep and large burns, the course of this type of injury becomes a general disease of the whole organism with the involvement of vital organs. The skin is the first to perceive the action of this exogenous factor, and secondarily there are changes in all body systems, in particular in the cerebellum. The aim of our study was to establish micro- and submicroscopic changes in the structural components of the cerebellar cortex one day after the experimental thermal injury. Simulation of experimental thermal injury was performed on white laboratory male rats. Grade III burns were applied under thiopental-sodium anesthesia with copper plates heated in boiled water to a temperature of 97-100°C. The size of the affected area was 18-20 % of the epilated body surface of rats. The cerebellum was collected after one day, further processing of the material for micro- and submicroscopic examination was performed according to accepted methods. Histological specimens were stained with methylene blue, and for electron microscopy the obtained ultrathin sections were contrasted with uranyl acetate and lead citrate according to the Reynolds method. At the micro- and submicroscopic levels, it was found that 1 day after the experimental thermal injury, reactive changes are observed in neurons, microcirculatory tract and glial cells. Thus, one day after the thermal trauma of the skin, the experimental animals showed the initial violations of the blood-brain barrier of the cerebellar cortex, which are adaptive-compensatory in nature. Initial, destructive, changes in neuro-glial-capillary relations, which occur primarily on their damage to the walls of hemocapillaries and are manifested by a violation of the micro- and ultrastructure of the main cells of the cerebellum – Purkinje, neurocytes of molecular and granular layers, neuroglyocytes, with peri-gliocyocytes were found

Анотація: Chronic obstructive pulmonary disease (COPD) is a widely spread disease, that can be prevented and treated. As it was mentioned in the GOLD guidelines, genetic factors have a prominent impact on the development of COPD. A great amount of different genes is involved into the development of COPD. They influence processes of inflammation, fibrosis and regulation of airways reactivity. Polymorphism of ADRB2 gene is of a particular interest as it is associated with the development of COPD and response to beta-2 agonists, which are the main drugs used in the treatment of COPD. The aim of our study was to investigate an impact of the polymorphism of ADRB2 gene on the clinical course of COPD. We collected source data, medical history in all patients in order to assess smoking status and smoking experience, clinical group of COPD according to GOLD classification, total amount of exacerbations, exacerbations treated in in-patient and out-patient conditions, data about use of antibiotics, glucocorticosteroids and methylxanthines. Blood was collected in all patients for the genetic analysis of ADRB2 gene polymorphism. Among COPD patients 65 (65 %) had changes in ADRB2 gene. 26 patients (26 %) had mutation and 39 patients (39 %) had polymorphism of ADRB2 gene, which indicate possible relation of ADRB2 gene with the COPD development. Patients of group 1, C79C allele carriers, had milder COPD course, which presented with the significant difference in the of GOLD D patients in this group (17.14±7.13 %) and group 2, C79G allele carriers, (58.97±8.15 %) and group 3, G79G allele carriers, (34.61±9.59 %). Group 1 patients had lower amount of exacerbations (2.543±0.281) when compared to group 3 (2.963±0.273), and lower amount of hospital admissions (1.031±0.154) when compared to group 2 (1.332±0.167). Group 1 patients also used less antibiotics then other groups. So, ADRB2 gene polymorphism in C79G positions is a prognostic factor of the severity of COPD course. It is associated with the greater amount of exacerbations and hospitalizations and also greater antibiotic use

Анотація: Hypertensive remodeling of the left ventricle (LV) is largely due to the influence of a number of control genes. In particular, the regulatory gene CYP11B2, which is responsible for the activity of aldosterone in blood plasma, affects the processes of cardiomyocyte hypertrophy, myocardial fibrosis and microcirculation. This study is devoted to the search for the effect of polymorphic aldosterone synthase carriers on the severity of the components of left ventricular hypertrophy in men with essential hypertension (EH) and representatives of the control group, residents of Podyllia region. The aim of the study was to determine associations between excess (inappropriate) myocardial mass, parameters of standard echocardiography and parameters of echoreflectivity in men with essential hypertension, carriers of different polymorphic variants of aldosteronesynthase gene. The study involved 150 men, aged 45-60 years, residents of the Podyllia region, who had no irreversible damage of target organs. Among them, 50 were in the control group, 58 – had EH of 1st stage and 42 men had EH of 2nd stage. All participants were measured for office blood pressure, performed a standard echocardiographic examination with the addition of standard EchoCG protocol by determination of the parameters of echoreflectivity and evaluation of appropriateness of left ventricular l mass (LVM) to hemodynamic load, according to the formula de Simone et al. and calculating the excessiveness ratio (ER) and determined the C-344T polymorphism of the CYP11B2 gene in venous blood samples by PCR. Statistical processing of the obtained results is performed using a specialized statistical application “Statistica 12.0”. It was found that the prevalence of CC polymorphism of the CYP11B2 gene in men with inappropriate LVM was almost twice higher than in men with appropriate to hemodynamic load LVM (p=0.015 by criterion χ2). At the same time, men with inappropriate LVM were characterized by higher values of echoreflectivity parameters BB and mCSV. In contrast to patients of the control group and patients with EH of 1st stage, patients with EH of 2nd stage, actual LVM (287.4 (53.9) g) significantly (p0.001) exceeded the predicted values (189 (37.8) g). According to the results of Spearman’s rank correlation analysis, it was found that the carrier of the CC genotype of aldosterone synthase gene is associated with higher values of the LVM ER. Thus, patients carrying the polymorphic CC variant of CYP11B2 gene are characterized by more pronounced cardiomyocyte hypertrophy, greater excess of LV mass relative to individual hemodynamic needs, more aggressive processes of myocardial fibrosis

Анотація: Modern cardiology requires the search for specific pathogenetically involved gene mutations, the consequences of which can be considered in the management of patients with hypertension. Scientists are targeting C/A polymorphism at the rs950880 position, which is associated with tissue expression of the IL1RL1 gene and the plasma level of soluble ST2 – a new biomarker in the diagnosis of cardiovascular disease. The aim of the study was to evaluate the association of rs950880 polymorphism of the IL1RL1 gene and the state of central and intracardiac hemodynamics in men with essential hypertension (EН) of varying severity, residents of the Podillia region of Ukraine. 170 men who met the inclusion criteria were examined according to a standard protocol, which included clinical, laboratory and instrumental examinations in accordance with current recommendations. The subjects were divided into a control group of 70 men without cardiovascular disease and a study group of 50 men with asymptomatic EН and 50 people with EH complicated by IIA stage chronic heart failure (CHF). Genotyping of SNP rs950880 of the IL1RL1 gene was performed using an allele-specific polymerase chain reaction. All men in the control group and the study group underwent echocardiography with Doppler according to the standard protocol. Statistical processing of the obtained results was performed in the package Statistica 12.0 using conjugation tables analysis, analysis of variance. It was found that among men living in Vinnytsia, Ukraine, carriers of СС and CA SNP rs950880 of the IL1RL1 gene dominate (42.35 % and 45.30 % of individuals, respectively), AA homozygotes are significantly less common (12.53 %, p0.05). Men without cardiovascular diseases and patients with EH do not differ significantly in the frequency of different variants of the genotype of the studied gene. C\A polymorphism is not associated with the risk of EН. The homozygotes AA with EH have a significantly lower LV myocardial mass index (LVMMI) (69.14±6.90 g/m2.7, compared with homozygotes CC – 75.42±2.54 g/m2.7, and heterozygotes CA – 76.96±3.18 g/m2.7, p0.05). Анотація: Among N-(4-methoxybenzyl)-4-methyl-2.2-dioxo-1H-2 λ6.1-benzothiazine-3-carboxamide derivates, the compound methoxybenzyl-amide derivative 4-methyl-2.2-dioxo-1H-2λ6.1-benzothiazine-3-carboxylic acid (compound NI-9) with pronounced analgesic and anti-inflammatory activities, which was superior to those of diclofenac and lornoxicam in the model of carrageenan edema. The aim of the study was to investigate the analgesic effect of benzothiazine-3-carboxamide derivative on different models of somatic and neuropathic pain syndromes. The study was performed on 91 male Wistar rats. Compound NI-9 and reference drugs meloxicam, diclofenac and gabapentin were administered intragastrically at doses of 3, 5, 8 and 5 mg/kg, respectively, corresponding to their ED50 in analgesic activity. Acetic spasms in mice, a model of thermal irritation of the tail flick in rats, as well as adjuvant arthritis and diabetic polyneuropathy were selected as models of pain syndromes. The results were processed in the program STATISTICA 10.0 using non-parametric methods. The results showed that methoxybenzyl-amide derivative 4-methyl-2.2-dioxo-1H-2λ6.1-benzothiazine-3-carboxylic acid (compound NI-9) has a pronounced analgesic effect on various models of pain syndromes, both somatic and inflammatory. and of neurogenic origin. The analgesic activity of the compound NI-9 in the model of acetic acid cramps in mice and thermal irritation in rats was 38.09 and 49.75 %, respectively, which was higher than that of meloxicam (36.73 and 45.68 %), and inferior to diclofenac (41.95 and 55.95 %). In the model of the systemic inflammatory process (adjuvant arthritis), the analgesic effect of NI-9 was statistically superior to meloxicam and diclofenac (43.32 % vs. 26.26 and 33.69 %). In a model of neuropathic pain syndrome (diabetic neuropathy), the analgesic effect of methoxybenzyl-amide derivative 4-methyl-2.2-dioxo-1H-2λ6.1-benzothiazine-3-carboxylic acid was greater than meloxicam (18.96 vs. 13.34 %), but this figure was lower than that of gaapentin (20.83 %). Further in-depth study of its pharmacodynamics and toxicity will be the theoretical basis for the development on the basis of this biologically active compound of the original drug with analgesic and anti-inflammatory activities
