Анотація: Determining importance in discussing the consequences of acute cerebrovascular accident (death, disability, complicated and severe course) is given to age, time since the onset of the disease, level of consciousness, severity of stroke, tactics and organization of medical and diagnostic process. The purpose of the work is to establish the dependence of medical and demographic indicators with risk factors for stroke and the organization of treatment and diagnostic process in patients of hospitals in Vinnytsia for the period 2017-2019, participating in the international program of the European Stroke Organization. The stroke register was created on the basis of the RES-Q report form of Vinnytsia hospitals, which are the most typical for the Podillia region of Ukraine. Estimation of correlations of medical and demographic indicators with unmodified factors of stroke, tactics of its management and condition of the patient at hospitalization was carried out by means of a statistical package "Statistica 5.5" with use of Pearson’s statistics. The main features of the studied correlations are: with unmodified stroke factors - regardless of sex, patients of both hospitals have inverse weak correlations age of patients with the ability to pass 10 m and direct weak correlations with mortality; with patient status on admission - regardless of sex, patients in both hospitals have reversed mean strength correlations of ability to pass 10 m with level of consciousness, NIHSS stroke severity and NIHSS stroke scores, and direct, mostly medium-strength correlations with scores on the Glasgow scale, and with a mortality rate established a symmetrically opposite picture of correlations, in addition, with the duration of treatment established multidirectional reliable weak strength correlations; with indicators of quality of medical care - regardless of sex, patients of both hospitals have weak correlations, the ability to walk 10 m with an indicator or hospitalized patient to stroke block and the appointment of antithrombolytics and direct weak force correlations with statins, with an indicator of treatment duration regardless from sex in patients of both hospitals established weak direct correlations with the appointment of antithrombolytic drugs, and with the mortality rate in both hospitals, more pronounced in men, established direct weak and medium strength correlations with the appointment of antithrombolytic drugs and inverse weak and medium strength correlations with the appointment of statins and antihypertensive drugs. Thus, numerous direct and inverse, mainly weak strength correlations of age, season, admission status and specifics of treatment and diagnostic measures with duration of treatment, functional outcome and stroke mortality for each hospital in general and individually in men and women of relevant medical institutions
Визначальне значення при обговоренні проблеми наслідків гострого порушення мозкового кровообігу (смерть, інвалідність, ускладнений та важкий перебіг) надається віку, часу з моменту виникнення захворювання, рівню свідомості, ступеню важкості інсульту, тактики і організації лікувально-діагностичного процесу. Мета роботи – встановити залежність медико-демографічних показників із факторами ризику інсульту та організації лікувально-діагностичного процесу у пацієнтів лікарень м. Вінниця за період 2017–2019 рр., що приймають участь в міжнародній програмі Європейського товариства інсульту. Інсультний регістр створено на основі форми звіту RES-Q лікарень м. Вінниця, які є найбільш типовими для Подільського регіону України. Оцінка кореляцій медико-демографічних показників із немодифікованими чинниками інсульту, тактикою його ведення та станом пацієнта при госпіталізації проведена за допомогою статистичного пакету «Statistica 5.5» з використанням статистики Пірсона. Головними особливостями досліджуваних кореляцій є: з немодифікованими чинниками інсульту – незалежно від статі у пацієнтів обох лікарень встановлені зворотні слабкої сили зв’язки віку пацієнтів з можливістю пройти 10 м і прямі слабкої сили зв’язки зі смертністю; з показниками стану пацієнтів при надходженні – незалежно від статі у пацієнтів обох лікарень встановлені зворотні середньої сили зв’язки можливості пройти 10 м із рівнем свідомості, важкістю інсульту за шкалою NIHSS і балами за шкалою інсульту NIHSS та прямі, переважно середньої сили зв’язки з балами за шкалою Глазго, а з показником смертності встановлена симетрично протилежна картина зв’язків, крім того, з тривалістю лікування встановлені різноспрямовані достовірні слабкої сили зв’язки; з показниками індикаторами якості медичної допомоги – незалежно від статі у пацієнтів обох лікарень встановлені слабкої сили зворотні зв’язки можливості пройти 10 м з показником чи госпіталізований пацієнт до інсультного блоку та призначенням антитромболітичних засобів і прямі слабкої сили з призначенням статинів, з показником тривалості лікування незалежно від статі у пацієнтів обох лікарень встановлені слабкої сили прямі зв’язки з призначенням антитромболітичних засобів, а з показником смертності в обох лікарнях, більш виражено у чоловіків, встановлені прямі слабкої і середньої сили зв’язки із призначенням антитромболітичних засобів та зворотні слабкої і середньої сили зв’язки із призначенням статинів і антигіпертензивних препаратів. Таким чином, встановлені багаточисельні прямі та зворотні, переважно слабкої сили, зв’язки віку, сезону, стану при надходженні та специфіки лікувально-діагностичних заходів із тривалістю лікування, функціональним результатом і смертністю від інсульту як для кожної лікарні загалом, так і окремо у чоловіків і жінок відповідних лікувальних закладів

Анотація: Carrying out comparative analysis of representative regional data on the level of diagnosis at the hospital stage of stroke management for a certain period of time and between different medical establishments allows us to form correctly a set of medical and preventive measures and to develop consistent recommendations for their improvement. The purpose: to assess the dynamics of quality indicators of clinical and hardware diagnosis of stroke for the period of 2017–2019 and to conduct a comparative analysis between the studied health care institutions. Materials and methods. According to the case histories, a comparative analysis was carried out on the level of organization of clinical and hardware diagnosis in the Municipal Non-Profit Enterprise “Academician O.I. Yushchenko Vinnytsia Regional Clinical Psychoneurological Hospital of Vinnytsia Regional Council” (VRCPNH) and Vinnytsia City Clinical Emergency Hospital (VCCEH) in 2017–2019. Statistical processing of the results was performed using Statistica 5.5 package. Results. Significant differences were found in the number of VRCPNH and VCCEH patients who underwent evaluation of stroke severity on the National Institutes of Health Stroke Scale at hospital stage, neuroimaging in the first hours of the disease, ultrasound of the neck vessels, atrial fibrillation and dysphagia screening, consultations of speech therapist and physiotherapist. In each hospital, there was a positive trend for three years after the completion of all diagnostic measures and procedures. Conclusions. The results of the study confirmed the adequacy of the studied quality indicators and the feasibility of further implementation of the hospital registry method in other regions of Ukraine to assess and control the quality of care, including the analysis of the effectiveness of new technologies
Проведення порівняльного аналізу репрезентативних регіональних даних стосовно рівня діагностики на госпітальному етапі ведення інсульту за певний період часу i між різними лікувальними закладами дозволяє коректно сформувати комплекс лікувально-профілактичних заходів та розробити послідовні рекомендації щодо їх поліпшення. Мета дослідження: оцінити динаміку показників якості клініко-апаратної діагностики інсульту за період 2017–2019 рр. та провести їх порівняльний аналіз між досліджуваними лікувально-профілактичними закладами. Матеріали та методи. За даними історій хвороби виконано порівняльний аналіз рівня організації клініко-апаратної діагностики в комунальному некомерційному підприємстві «Вінницька обласна клінічна психоневрологічна лікарня ім. акад. О.І. Ющенка Вінницької обласної ради» (ВОКПНЛ) i Вінницькій міській клінічній лікарні швидкої медичної допомоги (ВМКЛШМД) у 2017–2019 рр. Статистичну обробку результатів здійснено за допомогою статистичного пакета «Statistica 5.5». Результати. Встановлено вірогідні відмінності у відсотковій частці пацієнтів ВОКПНЛ i ВМКЛШМД, яким на госпітальному етапі проведено оцінку ступеня тяжкості інсульту за Шкалою інсульту Національних інститутiв охорони здоров’я США, нейровізуалізацію в перші години захворювання, ультразвукове дослiдження судин шиї, скринінг фібриляції передсердь та дисфагії, консультацію логопеда й фізіотерапевта. У кожному медичному закладі відмічено позитивну динаміку за три роки за фактом проведення всіх діагностичних заходів і процедур. Висновки. Результати дослідження підтвердили адекватність вивчених індикаторів якості і доцільності подальшого впровадження методу госпітального реєстру в інших регіонах України для оцінки і контролю якості медичної допомоги, включаючи аналіз ефективності нових технологій

Анотація: Due to the analysis of a whole set of indicators of the quality of medical care for acute cerebrovascular accident (ACA), and above all, the identification of major shortcomings in the maintenance of stroke register, it is possible to reduce morbidity, mortality and disability from this nosology. The purpose of the work is to study and compare the structure and risk factors of stroke according to the hospitals of Vinnytsia for the period 2017-2019. The stroke register was created on the basis of medical documentation of hospitals of the municipal non-profit enterprise “Vinnytsia Regional Clinical Psychoneurological Hospital named after Academician O.I. Yushchenko of Vinnytsia Regional Council” (VRCPNH) and Vinnytsia City Clinical Hospital of Emergency Medical Services (VCCHEMS), which are the most typical for this region of Ukraine. Statistical processing of the results was performed using the licensed statistical package “Statistica 5.5” using parametric evaluation methods. Between the two medical institutions in Vinnytsia and in the dynamics of time there were significant differences in different age, sex, type of stroke contingent of patients. When comparing the data of the territorial-population register of stroke in hospitals in Vinnytsia in 2017-2019, it was found that in both studied institutions the percentage of young and middle-aged patients, especially men, increased. The percentage of young and middle-aged patients in VRCPNH is higher than in VCCHEMS. Modified risk factors, which are currently the most important medical and social problem, are analyzed. There were no significant differences in the dynamics of stroke in both hospitals. The percentage of patients who had a stroke in hospital has decreased over time. The meteorological dependence of stroke has been proved, which points to the importance of preventive measures for people at risk. The main shortcomings in the collection of anamnestic data are analyzed. Analysis of risk factors for stroke suggests that the disease is based on various causes
Завдяки реалізації аналізу цілого комплексу показників якості надання медичної допомоги при гострому порушенні мозкового кровообігу, і насамперед, визначення основних недоліків у веденні інсультного регістру, можливо скоротити захворюваність, смертність та інвалідність від даної нозології. Мета роботи – вивчити та порівняти структуру та чинники ризику мозкового інсульту за даними лікарень м. Вінниці за період 2017-2019 рр. Інсультний регістр створено на основі медичної документації стаціонарів Комунального некомерційного підприємства “Вінницька обласна клінічна психоневрологічна лікарня ім. акад. О.І. Ющенка Вінницької обласної ради” (ВОКПЛ) та Вінницької міської клінічної лікарні швидкої медичної допомоги (ВМКЛШМД), які є найбільш типовими для зазначеного регіону України. Статистична обробка результатів здійснена за допомогою ліцензійного статистичного пакету “Statistica 5.5” з використанням параметричних методів оцінки. Між двома лікувальними закладами м.Вінниця та в динаміці часу встановлені достовірні відмінності у різного за віком, статтю, типом інсульту контингенту пацієнтів. При порівнянні даних територіально-популяційного регістра інсульту в лікарнях м.Вінниця в 2017-2019 рр. встановлено, що в обох досліджуваних закладах збільшилась відсоткова частка хворих молодого і середнього віку, в особливості чоловіків. У ВОКПЛ, порівняно з ВМКЛШМД, більша відсоткова частка пацієнтів молодого та середнього віку. Проаналізовано модифіковані чинники ризику, які в даний час є найважливішою медико-соціальною проблемою. За типом інсульту в динаміці в обох лікарнях суттєвих відмінностей не відмічалось. Відсоткова частка пацієнтів, у котрих інсульт виник у лікарні, з часом зменшилась. Доведено метеозалежність інсульту, що вказує на важливість превентивних заходів для осіб даної групи ризику. Проаналізовано основні недоліки при зборі анамнестичних даних.
