Анотація: Modern experiments in the study of periodontal deseas are aimed at improving the effectiveness of innovative methods of early diagnosis, treatment and prevention of this disease in patients. The conducted analysis of the literature showed that the disorder of microcirculation occupies one of the leading places in the pathogenesis of periodontitis. Disruption of the microcirculatory bed, based on the dyscirculatory changes of the capillary blood flow is accompanied by a decrease in the intensity of blood filling of vessels, their spasm. These changes, in case of chronization, include tissue disorders of the ischemic nature, up to the development of a capillary stasis. This can lead to disorder in gas exchange, trophism and pathomorphological damage to periodontal tissues. This type of change is most typical for age-related, traumatic, vascular, and stress-induced models. Morphological lesions in the vessels, namely ischemic phenomena were most pronounced in them. Microscopically, this was manifested in the heterogeneity of the epithelial layer, the thickening of individual layers, the detachment of the epithelial lining of the mucosa. As for the basal layer, the most common was the vacuolization of cells, the appearance of a significant number of cells that lacked nuclei (pathology, indicating a decrease in functional activity, impaired regenerative properties of the cell), expansion of intercellular contacts, a sign of tissue edema. In the basal membrane (function - dissociation of the epithelium from the actual lining of the mucous membrane), collagen fibers were loose, hypertrophic, their hyperplasia was observed, neutrophil leukocytes, lymphocytes, histocytes were found between the fibers, this testified to changes in cells. Some models lack a clinical picture of chronic hyperplastic periodontitis. The disorders consisted of changes in normal metabolism in periodontal tissues, decreased immune function, changes in morphological structure without induction of the inflammatory process
Сучасні експерименти в дослідженні захворювань пародонту спрямовані на підвищення ефективності інноваційних методів ранньої діагностики, лікування та профілактики цього захворювання в пацієнтів. Проведений аналіз літератури показав, що розлад мікроциркуляції посідає одне з провідних місць у патогенезі пародонтиту. Порушення мікроциркуляторного русла, що базується на дисциркуляторних змінах капілярного кровотоку, супроводжується зниженням інтенсивності кровонаповнення судин, їх спазмом. Ці зміни у випадку хронізації призводять до тканинних порушень ішемічного характеру, аж до розвитку капіляного стазу. Це може призвести до порушення газообміну, трофіки та патоморфологічного ураження тканин пародонта. Такий характер змін найбільш типовий для вікової, травматичної, судинної та стрес-індукованої моделей. Морфологічне ураження в судинах, а саме ішемічні явища, були найбільш виражені саме в них. Мікроскопічно це проявлялося в неоднорідності епітеліального пласту, потовщенні окремих шарів, відшаруванні епітеліального покрову слизової оболонки. Що стосувалося базального шару, то найпоширеніше виявилась вакуолізація клітин, поява значної кількості клітин, у яких були відсутні ядерця (патологія, що вказує на зменшення функціональної активності, порушення регенеративних властивостей клітини), розширення міжклітинних контактів, ознака набряку тканин. У базальній мембрані (функція – відмежовування епітелію від власне пластинки слизової оболонки) колагенові волокна були рихлі, гіпертрофічні, спостерігалась їх гіперплазія, між волокнами виявляли нейтрофільні лейкоцити, лімфоцити, гістоцити, що свідчили про зміни клітинного імунітету. У деяких моделей відсутня клінічна картина хронічного генералізованого пародонтиту. Порушення полягали в змінах нормального обміну в тканинах пародонта, зменшенні імунної функції, змінах морфологічної структури без індукції запального процесу

Анотація: Heat shock protein 60 (Hsp60) is considered as one of the possible autoantigens involved in the pathogenesis of a number of chronic diseases including periodontal diseases. The application of hyaluronic acid or hyaluronan (HA) in the treatment of periodontitis has been evaluated in several clinical trials, however, the effect of hyaluronic acid on heat shock protein 60 level in periodontal soft tissues has not been studied. The aim of this work was to evaluate the effect of HA injections on levels of Hsp60 in periodontal tissue of the rats. Samples of periodontal tissue of mandibular incisors of adult male Wistar rats at 10-12 months of age were investigated. Rats were distributed into the control group and the periodontitis group. Visual manifestations of hyperemia of the gums around the incisors were the criterion for selecting animals into the periodontitis group. There were two subgroups in the control group: intact rats (I); intact rats after HA “hyaDENT BG” 1.0 MDa (BioScience GmbH, Germany) treatment (I+“G-1.0”). There were four subgroups in the periodontitis group: rats with periodontitis (P); rats with periodontitis after HA “hyaDENT BG” 1.0 MDa (BioScience GmbH, Germany) treatment (P+“G-1.0”); rats with periodontitis after HA “SERTOBEC” 2.4 MDa (S.C. Rompharm Company S.R.L., Romania) treatment (P+“S-2.4”); rats with periodontitis after HA “SERTOBEC Tendon” 2.4 МDа (S.C. Rompharm Company S.R.L., Romania) treatment (P+“ST-2.4”). There were three animals in each subgroup. Rats were injected 0.05 ml HA in the area of alveolar processus of central incisors once a week, three times. Levels of Hsp60 in total lysates of periodontal tissue were tested by Western blotting method before and after the treatment with HA (one month after the last injection). There was no significant difference between levels of Hsp60 in total lysates of periodontal tissue of intact rats and rats with periodontitis before treatment (p0.05). Rats with periodontitis showed decreased inflammation in the periodontal tissue after treatment with HA with different molecular weight. Intact rats and rats with periodontitis which were treated with HA “hyaDENT BG” 1.0 MDa showed reduced levels of Hsp60 in total lysates of periodontal tissue comparatively with levels of Hsp60 before treatment (by 15.4 and 10.7 times respectively, p0.001). Rats with periodontitis which were treated with HA “SERTOBEC” 2.4 MDa or HA “SERTOBEC Tendon” 2.4 MDa also showed reduced levels of Hsp60 in total lysates of periodontal tissue comparatively with levels of Hsp60 before treatment (by 21.3 and 16.4 times respectively, p0.001). Rats with periodontitis showed the decrease in inflammation in periodontal tissue after treatment with HA with different molecular weight. Injections of HA has contributed to reduce levels of Hsp60 in periodontal tissue of intact rats and rats with periodontitis

Анотація: The problem of diagnosis and effective treatment of one of the most common complications of dental caries, such as pulpitis, including pediatric patients, is still quite relevant at the present time. Considering anatomy-physiological and age-related features of temporary and permanent teeth in children, the rapid and aggressive course of the carious process in poorly mineralized hard tissues due to the untimely and incomplete “maturation” of them, it is beyond any doubts that scientists and practitioners involved in pediatric dentistry deal with such complications in the clinic much more often than when dealing with adults. A particularly risky group are those who develop decompensated caries, according to classification by T.F. Vinogradova. Therefore, the search for more advanced methods of diagnosis and treatment of both acute and chronic inflammation of pulp, especially with the preservation of its viability, is logical and relevant in modern dentistry. Existing methods for the biological treatment of pulpitis have their positive aspects, but possible complications up to necrosis or pulp gangrene in remote terms indicate the need for improvement both the choice of a more effective method with clear indications for its use, and the technology and material of odontotropic direction. The aim of the work was to study the results of treatment of pulpitis in experimental animals by using tricalcium silicate cement for its further application in the clinic. An experimental study was conducted on 18 animals (9-month-old male rabbits, Dutch breed) in vivarium setting to obtain information on the morphogenetic mechanisms of regeneration of living tissues and cells of dental pulp in which traumatic pulpitis was induced artificially. By the nature of odontotropic material, four groups of experimental animals were divided into: group I (control) – traumatic pulpitis was treated without special odontotropic paste, group II - treatment of pulpitis with material based on calcium hydroxide, group III - material with trioxide aggregate, group IV - material with tricalcium silicate. Every two, four, and six weeks from the moment the animals were introduced into the experiment, the damaged teeth were removed and treated in a certain way and, after special treatment, were amenable to histological examination. In the pulp tissue, the state of its main components was studied: stroma, cellular elements and blood vessels of the microvasculature. After excretion of animals from the experiment on the 2nd and 6th weeks tissues of tooth were fixed in 10% formalin. Fixed in formalin tissue has been routine proceeding after decalcification in acetic acid with making histological slides which were stained with hematoxylin and eosin (H&E), according to van Gieson, Mallory, PAS-reaction was performed. The slides were studied with the microscope “Olympus BX-41” and followed interpretation by “Olympus DP-soft version 3.2”, which was used for morphometric study. Statistical comparison was performed using Mann-Whitney test for statistical analysis. The accepted level of significance was p0.05. Studying the state of the pulp in the phase of acute traumatic pulpitis in dynamic observation for a month and half period, at the end of the second, fourth and sixth week the possibility of a stable restoration of the viability of the pulp as well as all its components was clearly proven. If at the beginning of a morphological study, pulp injury was accompanied by clear signs of aseptic inflammation, both cellular, vascular and stromal formations, then after a month and a half there were signs of a clear restoration of pulp viability in the biological treatment of artificial pulpitis of the second, third and fourth study groups. The most significant dynamic signs of microscopic formations of the pulp were noted in animals of the forth study group treated with odontotropic paste with tricalcium silicate. The ability of this paste to complete tissue restoration in a short time period was provn. Therefore, it should be noted that artificially induced pulpitis in experimental animals is accompanied by significant breakages in the viability of the main components of pulp tissue, such as stromal fibers, microvasculature vessels and cellular elements. Filling and odontotropic materials in pastes for capping of the damaged pulp, such as calcium hydroxide, trioxide aggregate and tricalcium silicate, have full potential for restoring the viability of injured pulp, but with varying degree of effectiveness. According to histological research, biological material with tricalcium silicate is the most effective one for restoration of the pulp in a short time period
