Аннотация: lung neuroendocrine neoplasms embrace rather heterogeneous and rare malignancies which are usually characterized by nonspecific, “blurred” clinical signs thus complicating correct diagnosis or seriously delays it. Pulmonary neuroendocrine neoplasms accurate diagnostics and classification need to be improved. Histological examination should be supplemented by immunohistochemical tests to verify the neuroendocrine component, assess proliferative index of tumor cells, and confirm its bronchopulmonary origin. Immunohistochemistry is especially important in case of small or crushed biopsies, which account more than 50% of all specimens in lungs neuroendocrine neoplasms. Modern classification of lung neuroendocrine neoplasms and their grading are based on morphological criteria. Immunohistochemical markers expression is quite variable in different histological subtypes of bronchopulmonary neuroendocrine neoplasms, often data are descriptive, and correlation with morphology is studied insufficiently. The aim of this study was to define any significant correlation between different immunohistochemical markers expression, necrosis, proliferative index (Ki-67 ratio), and tumor grade in broncho-pulmonary neuroendocrine neoplasms. Histological blocks of lung neuroendocrine neoplasms from 113 unique patients (36 resections and 77 biopsies (54.5% of biopsies appeared to be small or crushed) were used in this study. The sample comprised 91 male and 22 female patients; the mean age was 59.2, CI 95% (56.9–61.4) years (from 19 to 77 years). Histological examination (including neuroendocrine morphology, necrosis, and grade) was provided in all cases. Also, immunohistochemistry, using Chr A, Syn, CD56, TTF-1, CK7, and Ki-67 before chemotherapy was performed. All morphological and immunohistochemical data were assessed by two different independent pathologists without the access to patient’s clinical data. All the observations were classified based on 2021 WHO Thoracic Tumors Classification. The sample was censored. We used nonparametric statistics (Spearman’s rank correlation) for this study. In was found that Chr A expression strongly (p0.05) correlated with immunohistochemical markers of primary lung malignancies (TTF-1 and CK7) that are mainly expressed in highly and moderately differentiated neuroendocrine neoplasms. Also, positive expression for TTF-1 and CK7 correlated with each other (p0.01). There was a strong negative correlation (p0.05) between Chr A staining and necrosis presence and it’s severity; between Chr A expression and tumor cells proliferation (Ki-67 ratio) (p0.01); and between Chr A labeling and tumor grade (p0.01). The correlation of immunohistochemical markers expression with necrosis, Ki-67 ratio and tumor grade was significant only for Chr A. All other tested options, for other markers were not statistically significant. It was defined that decrease or loss of Chr A expression reliably indicates tumors progression. Chr A expression can be used as an additional tool for grading of lung neuroendocrine neoplasmsНейроендокринні новоутворення легень включають досить гетерогенні та рідкісні пухлини, для яких зазвичай характерні неспецифічні, «стерті» клінічні прояви, що ускладнює встановлення правильного діагнозу або спричиняє суттєву його затримку. Коректна діагностика та класифікація нейроендокринних новоутворень легень потребують вдосконалення. Гістологічне дослідження має бути доповнене імуногістохімічними тестами з метою верифікації нейроендокринного компоненту, оцінки проліферативного індексу клітин пухлини, та підтвердження її бронхо-легеневого походження. Імуногістохімічне дослідження є особливо важливим у випадку малих або краш-біопсій, які складають понад 50% усіх зразків при нейроендокринних новоутвореннях легень. Сучасна класифікація нейроендокринних новоутворень легень та визначення ступеня їх диференціювання здійснюються за морфологічними критеріями. Експресія імуногістохімічних маркерів є досить варіабельною у різних гістологічних підтипах бронхо-легеневих нейроендокринних новоутворень, часто дані є описовими, їх кореляція з морфологічними ознаками вивчена недостатньо. Метою даного дослідження було визначення статистично значущої кореляції між експресією різних імуногістохімічних маркерів та некрозом, проліферативним індексом (рівень Ki-67) і ступенем диференціювання бронхо-легеневих нейроендокринних новоутворень. В дослідженні використані гістологічні блоки нейроендокринних новоутворень легень 113 пацієнтів — матеріал 36 резекцій і 77 біопсій (54,5% біопсій виявилися малими або роздробленими). Вибірка складалася з 91 пацієнта чоловічої статі та 22 жіночої; вік хворих від 19 до 77 років, у середньому 59,2 року ДІ 95% (56,9–61,4). Гістологічне дослідження (включаючи визначення нейроендокринної морфології, некрозу, ступеня диференціювання пухлини) проведене в усіх випадках. Також проведене імуногістохімічне дослідження до призначення хворим хіміотерапії (з використанням маркерів Chr A, Syn, CD56, TTF-1, CK7 та Ki-67). Всі морфологічні та імуногістохімічні дані оцінювалися двома незалежними патологами без доступу до клінічних даних пацієнта. Всі спостереження класифіковані за WHO Thoracic Tumors Classification (2021 р.). Вибірка була цензурованою. У досліджені використані методи непараметричної статистики (рангова кореляція Спірмена). Виявлений сильний кореляційний зв’язок (p0.05) між експресією Chr A та імуногістохімічних маркерів первинних пухлин легень (TTF-1 і CK7), які переважно відзначають у високо та помірно диференційованих нейроендокринних новоутвореннях. Також виявлена позитивна кореляція між експресією маркерів TTF-1 і CK7 (p0.01). Визначений сильний негативний кореляційний зв’язок (p0.05) між реакцією на Chr A та наявністю і вираженістю вогнищ некрозу; між експресією Chr A і проліферацією пухлинних клітин (рівень Ki-67) (p0.01); а також між Chr A та ступенем диференціювання пухлини (p0.01). Кореляційний зв’язок між експресією імуногістохімічних маркерів, некрозом, рівнем Ki-67 та ступенем диференціювання пухлини виявився статистично достовірним лише для Chr A. Решта перевірених комбінацій показників, для інших маркерів не були статистично значущими. Визначено, що зниження або втрата експресії Chr A є надійним свідченням пухлинної прогресії. Експресія Chr A може бути використана як додатковий інструмент для розподілу нейроендокринних новоутворень легень за ступенем злоякісності

Аннотация: Recent advances in the treatment of breast cancer (BC) have been related to the personalization of therapy. The methylation status of the promoter regions of tumor suppressor genes such as BRCA1 and BRCA2 is supposed to be useful as a prognostic factor in BC patients. Aim: To investigate the frequency of hypermethylation in the promoter regions of BRCA1 and BRCA2 genes in tumor tissue of BC patients, and the relation of hypermethylation to the clinical course of the disease. Materials and Methods: Molecular genetic studies were performed on 50 BC tissue samples in order to determine the methylation status of the promoter regions of the BRCA1 and BRCA2 genes. Results: Hypermethylation of the BRCA1 promoter region was detected in 34% of BC cases, hypermethylation of the BRCA2 promoter region — in 50% of cases, and hypermethylation of the promoter region of both genes — in 20% of cases. A significant increase in the incidence of hypermethylation of the BRCA2 promoter region was found in the group of patients older than 56 years, mainly in patients with triple-negative breast cancer and without family history of BC. Conclusions: The high frequency of hypermethylation in the promoter regions of BRCA1 and BRCA2 genes, as well as their co-methylation in tumor tissue of BC patients has been detected Останні досягнення в лікуванні раку молочної залози (РМЗ) пов’язані з персоналізацією терапії. Вважають, що статус метилування промоторних ділянок генів-супресорів пухлин, таких як BRCA1 та BRCA2, може бути інформативним як прогностичний фактор у хворих на РМЗ. Мета: Оцінити частоту гіперметилування промоторних ділянок генів BRCA1 та BRCA2 в пухлинній тканині хворих на РМЗ, а також зв’язок гіперметилування з клінічним перебігом захворювання. Матеріали та методи: У 50 зразках пухлинної тканини молочної залози провели молекулярно-генетичні дослідження з метою визначення статусу метилування промоторних ділянок генів BRCA1 та BRCA2. Результати: Гіперметилування промоторної ділянки гена BRCA1 було виявлено в 34% випадків РМЗ, гіперметилування промоторної ділянки гена BRCA2 — у 50% випадків, а гіперметилування промоторних ділянок обох генів — у 20% випадків. Значне підвищення частоти гіперметилування промоторної ділянки гена BRCA2 було виявлено в групі хворих віком старше 56 років, в основному у хворих з тричі негативним РМЗ та без обтяженого сімейного анамнезу РМЗ. Висновки: Виявлено високу частоту гіперметилування промоторних ділянок генів BRCA1 та BRCA2, а також їх кометилування в пухлинній тканині хворих на РМЗ

Аннотация: Мета роботи – оцінювання зв’язку мітотичного індексу (MI), некрозу, імуногістохімічних (ІГХ) маркерів проліферації Ki-67, PHH3 та їхньої цінності як предикторів агресивності нейроендокринних пухлин (НЕП) легень. Матеріали та методи. Дослідження здійснили, використавши операційний і біопсійний матеріал, що отримали від 64 пацієнтів із НЕП легень до призначення їм хімієтерапії. Виконали морфологічне та ІГХ дослідження. MI, вираженість некрозу, експресію Ki-67 і PHH3, метастатичні ураження та виживаність хворих оцінювали за допомогою методів непараметричної статистики. Результати. Виявили статистично вірогідний зв’язок між вираженістю некрозу та виживаністю пацієнтів (р = 0,021). Це стосувалося порівняння груп хворих без вогнищ некрозу у тканині пухлини та з екстенсивними вогнищами некрозу (p = 0,023). MI достовірно пов’язаний із наявністю метастазів у лімфатичних вузлах (p = 0,003) і віддалених метастазів (p = 0,029). Визначили значущий прямий зв’язок між рівнем Ki-67 і MI (р 0,001), MI та експресією PHH3 (p 0,001). Але не виявили вірогідний зв’язок між рівнями Ki-67 і PHH3 (p

Аннотация: To determine the prognostic value of tumor-infiltrating lymphocytes (TILs) in colon adenocarcinomas. Materials and Methods: The study was performed on 180 paraffin blocks of operation material from patients diagnosed with adenocarcinoma of the colon, treated at the Kyiv City Oncology Center in 2013–2018. TILs were counted on histological slides stained with hematoxylin-eosin. By TILs count percentage in tumor slides, the samples were divided into three groups: 0–9% TILs (n = 65); 10–39% TILs (n = 79); and 40% TILs (n = 36). Results: Kaplan — Meyer estimate showed that the difference in overall survival between groups was significant (p = 0.001). Multivariate Cox’s proportional hazard regression model analysis evidenced on significantly better overall survival rates in groups with moderate TILs percentage (hazard ratio 0.54, 95% CI 0.30–0.97, p = 0.042) and high TILs percentage (hazard ratio 0.36, 95% CI 0.13–0.99, p = 0.049), respectively, as compared with low TILs percentage group. Conclusion: TILs content can be considered as an independent prognostic factor for colon adenocarcinoma and used as an additional tool in routine practice of pathologists

Аннотация: Parathyroid carcinoma (PC) is an uncommon cause of primary hyperparathyroidism (pHPT). The tumor is mostly functioning with following high serum calcium and parathyroid hormone (PTH) levels. Consideration of PC in differential diagnosis of pHPT is important because the mortality and the prognosis depend on early recognition and surgical approach. A case of PC in 51-year old female patient with persistent pHPT is reported. A patient, referred with anterior cervical hematoma, dysphagia, underwent neck ultrasound, barium X-ray swallow test and neck computed tomography. On the posterior surface of left thyroid lobe, close to the esophagus the heterogenic lesion with irregular margins was found. The preoperative calcemia was slightly elevated. The patient underwent cervical exploration with parathyroidectomy. Histological exam showed parathyroid lesion with immunohistochemical confirmation of PC. After 2 months follow-up there are no signs of a residual-recurrent disease. Although laboratory tests revealed pHPT preoperatively, no clinical effects of elevated calcium and PTH concentration levels were presented. As histological features are not specific for the differential diagnosis between the parathyroid adenoma and PC, the immunohistochemistry remains the only useful tool for the definite diagnosis

Аннотация: Tumors of the heart are uncommon and usually benign (in 93% cases myxomas are observed). More often secondary, metastatic tumors are detected in the heart, as a rule, at pronounced progression of the malignant neoplasm with multiple lesions of other internal organs (lung, pleura, liver, etc.). Literature review on cardiac metastases of different tumors is given. Case Report: Own observation of a young man with rare single metastasis of malignant testicular germ cell tumor with predominance of embryonic carcinoma in the right ventricle of the heart is presented; the primary tumor was detected after metastasis revealing. The diagnostic algorithm using routine histological study supplemented with immunohistochemistry, including detection of cytokeratin pan, cytokeratin 5/6, cytokeratin 7, CD30, OCT4, TTF-1, hCG, and AFP markers expression, is described
