Аннотация: A pineal gland attracts much attention of scientists lately, because it secrets melatonin, which is a very important hormone. Melatonin plays a significant role in the development of pregnancy: it enhances implantation, decreases oxidative stress etc. At the same time, the links between the pineal gland and the placenta, as a part of endocrine system of a mother, are still not well described. Objective of the study. To investigate the pathogenic links between secretion of melatonin, placental growth factor and reproductive hormones in pregnant women with placental insufficiency, manifested as intrauterine fetal growth restriction. Material and methods. 35 pregnant women aged 18-36 with placental insufficiency (PI) were examined (study group). The placental insufficiency manifested as the intrauterine fetal growth restriction (IUGR) in the 3rd pregnancy trimester. The control group consisted of 20 women with uncomplicated pregnancy at the same term. The blood concentrations of melatonin and placental growth factor (PlGF) were studied, as well as the blood concentrations of certain placental hormones: progesterone, placental lactogen and unconjugated estriol. Results. The concentration of melatonin was found to decrease significantly, if pregnancy was complicated by intrauterine fetal growth retardation (study group – 129.90 ± 17.65 pg/ml, control group – 231.25 ± 21.56 pg/ml, p 0.01), as well as concentration of PlGF (study group – 130,78 ± 15,80 pg/ml, control group – 230.0 ± 29.97 pg/ml, p 0.01). A significant difference of progesterone concentrations between the groups was found (study group: 15.36 ± 2.78 ng/ml, control group: 30.43 ± 2.66 ng/ml, p 0.01), as well as for placental lactogen (study group: 6.31 ± 2.08 mg/l, control group: 7.76 ± 1.93 mg/l, p 0.05). Аннотация: The pathogenetic theory of retrograde outflow of endometrial cells into the peritoneal cavity at endometriosis is gaining an increasing support. Adequate blood supply and angioneogenesis play an important role in successful implantation and occurrence of ectopic foci. VEGF plays an important role in this process. Determination of the pathways affecting the activity of this factor seems to be promising in terms of its effect produced on the early pathogenetic links of endometriosis. Objectives: to determine the effectiveness of dopamine agonist cabergoline and highly selective COX-2 inhibitor celecoxib in an experimental model of external genital endometriosis in rats. Methods. 83 outbred white female rats of Rattus Norvegicus Wistar were involved in the experiment. Experimental induction of endometriosis was performed by surgery and implantation of the autologous uterine fragments. The type and volume of experimental endometriosis lesions on the peritoneum of experimental animals were evaluated macroscopically as well as their hystologic examination were performed. Results. Administration of a dopamine receptor agonist as a VEGF inhibitor separately was found to produce a pronounced inhibitory effect on ectopic endometrioid formation. However, the use of a dopamine receptor agonist in combination with a highly selective COX-2 inhibitor does not lead to potentiation or summation of their effects. At the same time, the use of COX-2 inhibitor alone has shown significantly lower efficacy than using the dopamine receptor agonist as a VEGF inhibitor. Conclusion. At experimental endometriosis in rats, dopamine agonists and highly selective COX-2 inhibitors were found to be one of VEGF inhibitors available

Аннотация: The pineal gland produces the important hormone melatonin, the level of which in the blood of pregnant women decreases in case of placental insufficiency. The effect of pineal dysfunction on the immune system of pregnant women and the angiogenic activity of the placenta during pregnancy remains insufficiently studied. Objective: to study the effect of our method of non-drug correction of pineal function on the state of the cytokine part of the immune system and the synthesis of placental growth factor (PlGF) in pregnant women with placental insufficiency manifested as fetal intrauterine growth restriction (IUGR). Material and methods. 46 pregnant women with IUGR at 30-36 weeks of gestation were examined. The group was divided into two subgroups: with non-drug correction of the pineal function (n = 25) and without correction (n = 21). The method of correction included a set of measures to follow the lighting regimen, activity and sleep for 14 days. The control group consisted of 20 women with uncomplicated pregnancy. Levels of melatonin, PlGF, TNF-α, IL-1β, IL-6, IL-4, IL-10 were determined in the venous blood by enzyme-linked immunosorbent assay. Results. It was established that the concentration of melatonin in the blood of pregnant women with IUGR was significantly reduced, as well as the concentration of PlGF (p 0.01). Significant changes were also found in pregnant women with placental insufficiency, namely, increased concentrations of proinflammatory cytokines, such as TNF-α (p 0.05), IL-1-β (p 0.001) and IL-6 (p 0.05), comparing to healthy pregnant women. Also, in the group of pregnant women with IUGR, the levels of anti-inflammatory cytokines IL-4 (p 0.001) and IL-10 (p 0.001) were elevated in comparison to the control group. After application of the developed complex of non-drug correction of pineal function, the concentration of melatonin in the blood of pregnant women in the subgroup of correction increased significantly, comparing to the subgroup without correction (p 0.001), as well as the level of PlGF (p 0.05). Also, significantly lower levels of proinflammatory cytokines TNF-α, IL-1-β and IL-6 were observed in pregnant women in the subgroup of correction (p 0.01). Regarding anti-inflammatory cytokines, there was a decrease in the level of IL-4 and an increase in the level of IL-10 (p 0.01) under the influence of the non-drug correction. Conclusions. When the measures, aimed at non-drug correction of the pineal function, are applied in pregnant women with placental insufficiency, manifested as IUGR, the following changes are observed: increased plasma levels of melatonin and placental growth factor, decreased levels of proinflammatory cytokines. We suggest that the pineal gland exerts its effect on the immune system through melatonin, which moderates the activity of pro- and anti-inflammatory cytokines. Therefore the influence of inflammation on placental tissue reduces and results in the increase in the concentrations of placental growth factor in the blood of pregnant women
